Choir Field Trip Permission

The Holy Name School Choir will be performing at the Indianapolis Marriot for the Indiana Electric Cooperative's annual conference. We perform during the luncheon. Students should wear their school uniform for the event. Out-of-uniform privileges do not apply.
We will leave Holy Name at approximately 11:30am and travel by bus to the Indianapolis Marriot Downtown. We will setup and perform from approximately 12:30-1:00pm. Following the performance students will receive a box lunch and an electrical safety presentation. We will then board the bus and return to Holy Name by approximately 2:30pm.
Permission Form

I request that Holy Name Catholic School allow my child to participate in the field trip to the Indianapolis Marriot Downtown on December 9, 2024. I hereby release and save harmless Holy Name of Jesus Catholic School and any/all its employees from any and all liability of any/all harm arising to my child. I, the undersigned, have read this release and I understand all its terms and execute it voluntarily and with full knowledge of its significance.

In the event of an emergency in which we cannot be contacted, we hereby authorize that emergency treatment may be administered.
